As Care Coordinator for the DOTT program, your main job purpose is to provide overall case management. This includes, but is not limited to, providing outreach services, life skills training, crisis intervention, resource referral management, community networking, and resource support of the clients to decrease their chance of recidivism in crisis services or the child welfare system. Daily Responsibilities include looking into community resources, networking, and maintaining current information resources, improving client’s engagement in treatment, assisting clients to obtain and maintain resources, and maintaining necessary paperwork to document those services. In addition, Care Coordinators must show competency in effectively engaging individuals undergoing treatment services. They must also show an ability to document in a clear and concise manner to maintain compliance with DCF, CARF, and programmatic requirements. All functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.

Requirements:

Applicant must have:

  • Bachelor’s Degree in a Human Services field (i.e., Psychology, social work, etc),
  • Ability to receive and maintain a Level II Background clearance,
  • Acceptable Motor Vehicle Registration driver’s license record in accordance with the guidelines set by Aspire insurance company, and
  • Preferred: Case Management experience with clients with substance use disorders and/or mental health disorders.
